Important Information About Your Appointment
Every patient will be asked to provide a range of information when calling the Canberra Orthopaedic Group to request a consultation. The following points have been provided to assist you in providing the necessary information.
Doctor's letter of referral: you must have a letter of referral from your general practitioner in order to make an appointment with our orthopaedic surgeons.
Health insurance: Our specialist orthopaedic surgeons conduct a private practice, and details of your health insurance will be required when making an appointment. For those patients who choose to be treated in the public system, a number of our surgeons also conduct an out-patient clinic at the Canberra Hospital. Appointments for these clinics can be made through the Canberra Hospital on Tel: 6244 2415, or 6244 2071, or 6244 2406. You will also need a referral from your doctor when making an appointment at the out-patient clinics.
Workers Compensation & 3rd Party cases: You must have a letter of authority from the Workers Compensation or 3rd Party insurer approving your treatment and an authority number. The name of the contact person and address to forward the invoice to would also normally be included in this letter.
X-rays and scans: You need to bring with you to the consultation all x-rays and CT or other scans that relate to your current condition or injury.
Patient registration forms: You are encouraged to download and complete both of these forms prior to your appointment. Your completed forms can then be given to the COG reception staff at the time of your appointment. Alternatively, these forms are also available in COG rooms and you may complete them at the time of your appointment. You can download these forms here:

Consultation fees and payments: Individual fees for treatment and consultation may vary depending on length of consultation, area and type of treatment, the specialist surgeon you consult and the place of consultation. You will be informed of the exact fees for your consultation at the time you make a appointment with our staff.
